WAKE FOREST, N.C. — The federal government has again cited Wake Forest University for violations at its monkey research facility.

The Department of Agriculture said researchers didn't follow protocol and failed to maintain an enclosure.

A monkey died in one of the incidents.

The government said Wake Forest must re-train staff and set up a new maintenance process.

Wake Forest University School of Medicine released this statement in response to the citation:

We are committed to the highest standards of care that come with the privilege of working with animals. We were incredibly saddened by the accident involving a vervet. We provided immediate veterinary care, but unfortunately, those efforts were not successful. We have taken measures to help ensure an incident like this does not occur again.
